"Will I look frozen?" — what actually determines a natural result

If I had to pick the single question I hear most often — in clinic, in DMs, from friends — it's this one. Someone is curious about anti-wrinkle treatment, sometimes has been for years, but one fear keeps them from booking: "Will I look frozen? Will I still look like me?"

It's a completely reasonable fear. We've all seen faces that don't move. So let me give you the honest answer I give my own patients at our Limerick clinic.

The frozen look isn't caused by the treatment. It's caused by decisions.

Anti-wrinkle injections work by relaxing specific small muscles that repeatedly crease the skin. The medicine itself doesn't have a "frozen" setting — the result you get is entirely determined by three decisions made by the person treating you:

• How much is used. Higher doses relax muscles more completely. A conservative dose softens movement; an aggressive one eliminates it.
• Where it's placed. Facial muscles work as a system. Precise placement softens the crease-forming muscles while leaving the expressive ones alone. Careless placement doesn't.
• Whether your face was assessed as your face. Everyone's anatomy, muscle strength and expression habits are different. A template dose applied to every patient produces template problems.

In other words: the frozen look is a choice — usually a combination of over-treatment and under-assessment. Which means it's also entirely avoidable.

What a natural result actually looks like

When dosing and placement are right, here's what happens: the lines you were worried about soften. You still raise your eyebrows. You still frown at bad drivers. Your face still does everything it did before — it just stops etching the same creases so deeply while it does it.

The best description I've heard from a patient: "Nobody could tell, but everybody noticed." People said she looked well. Rested. Fresher. Not one person asked if she'd had something done.

My rule is simple: I would always rather under-treat and review than overdo it. You can add more two weeks later. You cannot take it back out. So at your first treatment, especially if you've never had anything done before, we start conservatively — soften rather than switch off — and review how your face responds.

And before any of that, there's a consultation. Not a formality — a real assessment of your anatomy, your goals, and whether treatment is even the right answer for what's bothering you. Sometimes it isn't, and I'll tell you that too. Sometimes the tired look people want to fix is really a skin-quality issue, and a completely different treatment serves them better.

Questions to ask any provider (including us)

• Who is actually performing my treatment, and what is their medical background?
• Will you assess my face individually, or is there a standard dose?
• What happens if I don't like the result?
• Are you comfortable telling patients "no" or "less"?

Any good clinic will welcome these questions. If a provider seems irritated by them — that tells you something useful too.
